Horizon Releaf is a medical marijuana certification practice in Temple Terrace, Florida. What is Horizon Releaf's mission?
Horizon Releaf exists to make medical marijuana certification in Florida straightforward and honest: a real exam with a real physician, clear prices before you book, and the time to answer your questions, whether or not you qualify.
What does it mean to be a qualified physician in Florida?
In Florida, only a qualified physician can certify a patient for medical marijuana. Before becoming a qualified physician, and before each medical license renewal, the physician must complete a 2-hour course and exam from the Florida Medical Association or Florida Osteopathic Medical Association.
Qualified physicians must also stay independent of the businesses that grow and sell marijuana. A qualified physician may not be employed by, or have any direct or indirect economic interest in, a medical marijuana treatment center or testing lab.
How do we practice?
We practice the way the law intends and the way we would want to be treated as patients. Three commitments guide every visit.
- No economic ties to dispensaries. Florida law forbids a qualified physician from being employed by, or having any economic interest in, a treatment center or testing lab. We do not recommend or refer to any particular dispensary or product.
- An in-person first visit. Before an initial certification, the physician must do an in-person physical exam, meaning the physician is physically in the same room as the patient. After that, existing patients can renew by telehealth.
- Clear pricing. The initial evaluation is $85, and renewals and transfers are $85. The state ID card fee is paid separately to the Florida Department of Health, and we tell you that before you book.
What should you expect at your visit?
Before your first visit, you complete a short medical history and the Florida consent form online. Bring a valid Florida driver license or ID and any records about your condition, such as office notes, imaging reports, or a medication list.
At the visit, the physician reviews your history and records, examines you, and talks through the risks and benefits for your situation. You have time to ask questions, and nothing is rushed.
If you qualify, the physician enters your certification in the state Registry, and you then apply for your ID card with the Florida Department of Health. If you do not qualify, you are told why.
What if I do not qualify?
The physician tells you honestly, and explains why. Certification is a medical decision based on Florida law and your health, not a purchase.
Whether or not you qualify, you leave with your questions answered. That is part of the visit.
Where is Horizon Releaf?
Our office is at 5340 E 131st Ave, Suite 106, Temple Terrace, FL 33617. All first visits take place here.
We see patients from across the Tampa Bay area. Existing patients can do their re-evaluations by telehealth, so they do not need to return to the office for renewals.

Do you accept insurance?
No. Health insurers and government agencies are not required to reimburse any expenses for the medical use of marijuana, and marijuana is not reimbursable under Florida workers' compensation. Visits are self-pay, and our prices are posted before you book.
