Patients from Hillsborough, Pasco, Pinellas, and Polk counties come there once for the in-person exam Florida requires. After that, existing patients can do their renewal evaluations with our physician by telehealth from anywhere in Florida.

Who does Horizon Releaf serve?
We serve Florida residents across the Tampa Bay area who want a medical marijuana evaluation. Many come from Tampa and Temple Terrace, and others drive in from Pasco, Pinellas, and Polk counties.
Every patient needs a qualifying condition and proof of Florida residency. Adults prove Florida residency with a valid Florida driver license or Florida ID card. Seasonal residents without one may submit two proof-of-address documents (such as a lease, deed, recent utility bill, or recent government or bank mail). Minors submit a birth certificate or Florida K-12 school record plus a parent or guardian's Florida license or ID.
Where do I go for my first visit?
Every first visit takes place at our Temple Terrace office. We do not have offices in any other city, and we do not hold visits at other sites.
Florida law requires this. Before an initial certification, the physician must do an in-person physical exam, meaning the physician is physically in the same room as the patient.
Can I renew by telehealth?
Yes, once you are an existing patient. Telehealth is allowed only for renewals: a physician who already certified the patient after an in-person physical exam may do that patient's later exams and re-evaluations by telehealth.
This is what makes a single office practical for patients who live across the bay or up I-4. You make one trip, and most later visits happen by video.

Do you have offices in Tampa, St. Petersburg, or Clearwater?
No. Horizon Releaf has a single office, in Temple Terrace. Patients from every city come there for the first in-person exam.
Can I get my first certification by video?
No. Florida law requires an in-person physical exam before an initial certification. Telehealth is only allowed for renewals of existing patients by the same physician.
Can I renew by telehealth if I moved farther away within Florida?
Yes. If our physician certified you in person, your later re-evaluations can be done by telehealth while you remain a Florida resident.
Does my county matter?
No. Florida's medical marijuana program and ID card are run by the state Department of Health, so any Florida resident can see any qualified physician in the state.
