Where exactly is the office?
We are at 5340 E 131st Ave, Suite 106, Temple Terrace, FL 33617. East 131st Avenue runs off North 56th Street, north of Fowler Avenue and just south of Fletcher Avenue.
This is our only location. Patients from every city we serve come to this suite for their first exam.
How do I get there from central Temple Terrace?
From the middle of the city, head to North 56th Street (State Road 583) and drive north across Fowler Avenue. Turn onto East 131st Avenue.
The trip we measured is about two miles and roughly five minutes with clear roads. Because 56th Street is the city's main north-south road, most local drivers already know the way.
- From Busch Boulevard or the south side of the city: North 56th Street north, past Fowler Avenue.
- From Fletcher Avenue: turn south on North 56th Street; 131st Avenue comes up quickly.
- From I-75: take the Fowler Avenue exit, go west to North 56th Street, then north.
What does living this close change for me?
Mostly, it makes scheduling easy. A local patient can book a first visit around a lunch break or a school pickup without planning a long drive.
It also means you have a choice at renewal time. You can come back to the office in person or use telehealth, whichever fits your week.
Why is the first exam in person?
Florida requires it for every new patient. Before an initial certification, the physician must do an in-person physical exam, meaning the physician is physically in the same room as the patient.
During the visit, the physician reviews your history and records and decides whether you have a qualifying condition under state law. You can only be certified if you do.
Why do patients from other counties come to Temple Terrace?
The office sits between two interstates. I-275 and I-75 both have Fowler Avenue exits, and each is only a few miles from our door.
That puts us within reach of Pasco drivers coming down I-75, Pinellas drivers crossing the bay on I-275, and Polk drivers arriving by I-4. Local residents simply skip the highway.
Can I still use telehealth even though I live nearby?
Yes. Telehealth is allowed only for renewals: a physician who already certified the patient after an in-person physical exam may do that patient's later exams and re-evaluations by telehealth. Distance does not decide it; being an existing patient of the same physician does.
Renewals are due at least once every 30 weeks. Some Temple Terrace patients like to come in for these, and others prefer a video visit from home.

If I live in Temple Terrace, can I choose telehealth for renewals?
Yes. Florida allows telehealth re-evaluations for existing patients of the same physician, no matter how close they live. You can also renew in person if you prefer.
What should I bring to my first visit?
Bring your Florida driver license or ID card and any medical records about your condition, such as office notes, imaging reports, or medication lists. Records help the physician confirm whether your condition qualifies.
Can I renew in person instead of by video?
Yes. Telehealth is an option for existing patients, not a requirement. Temple Terrace patients often find a quick in-person renewal just as easy.
