Tampa residents are certified at our one office in Temple Terrace, about 13 miles from downtown Tampa, or roughly 20 minutes without traffic by I-275 and Fowler Avenue. Florida law requires the first exam in person. After that, our physician can do your renewal evaluations by telehealth, so most later visits happen from home.

Is there a Horizon Releaf office in Tampa?

No. Our only office is in neighboring Temple Terrace, at 5340 E 131st Ave, Suite 106. Tampa patients come there for the first visit.

Temple Terrace shares a border with Tampa, and both cities sit in Hillsborough County. For many people in North Tampa the office is closer than downtown is.

How do I get from downtown Tampa to the office?

The most direct route from downtown is north on I-275. Take the Fowler Avenue exit (State Road 582) and head east.

Stay on Fowler Avenue for close to four miles, then turn north on North 56th Street. East 131st Avenue is a short way up 56th Street, between Fowler and Fletcher Avenues.

We measured the trip at about 13 miles and roughly 20 minutes with open roads. Rush hour on I-275 can stretch that, so plan extra time for a morning or late-afternoon visit.

  • From South Tampa or Westshore: pick up I-275 north and follow the same Fowler Avenue exit.
  • From New Tampa: Bruce B. Downs Boulevard south to Fletcher Avenue, then east to North 56th Street.

Why does the first visit have to be in person?

It is a state rule, not our preference. Before an initial certification, the physician must do an in-person physical exam, meaning the physician is physically in the same room as the patient.

Bring a valid Florida driver license or ID card and any records about your condition. The physician reviews them during the exam.

Can Tampa patients renew by telehealth?

Yes, once you are an existing patient. Telehealth is allowed only for renewals: a physician who already certified the patient after an in-person physical exam may do that patient's later exams and re-evaluations by telehealth.

A 20-minute drive does not sound long, but I-275 traffic through the city can double it. A video renewal lets you skip the commute, parking, and time away from work.

Renewals are due at least once every 30 weeks. We will remind you before your certification runs out.

Which parts of Tampa are closest to the office?

North Tampa is closest. Homes near Fowler and Fletcher Avenues can be just a few minutes from our door, closer than downtown is.

The University of South Florida's Tampa campus is a short drive from the office. Students and staff there can often reach us without touching the interstate.

South Tampa has the longest trip inside the city, because you cross downtown on I-275 first. Plan on more than the 20 minutes we measured from downtown if you start there at rush hour.

What happens after the Tampa-area evaluation?

If you qualify, our physician enters you in the Medical Marijuana Use Registry (MMUR), run by the Florida Department of Health, Office of Medical Marijuana Use. You then apply for your state ID card online.

After the physician enters you in the Registry, you apply online in the Medical Marijuana Use Registry (or by mail) with a photo, proof of Florida residency, an e-signature, and the $75 fee. Online applications average about 10 business days to approve. Since July 27, 2026, you can only apply or renew if you have a current or scheduled certification with open orders in the Registry.

The card is issued by the Florida Department of Health, so the process is the same whether you live in Tampa or elsewhere in the state.

How long is the drive from downtown Tampa?

Will I need to come back to Temple Terrace for every renewal?

Usually not. Florida lets the same physician do later re-evaluations by telehealth after the first in-person exam, so most Tampa patients renew from home.

Does it matter that I live in the city of Tampa rather than Temple Terrace?

No. Tampa and Temple Terrace are both in Hillsborough County, and the state card works the same for any Florida resident. What matters is proof of Florida residency and a qualifying condition.
