How much is a medical marijuana card in Florida?
At Horizon Releaf, your first year costs about $247.75 if you apply online: an $85 initial visit, the state ID card fee paid to the Florida Department of Health, and one $85 re-evaluation, since Florida requires a new physician evaluation on a set schedule. Health insurance isn't required to cover certification, so visits are self-pay.

What does a Florida medical marijuana card cost in the first year?
A first year at Horizon Releaf costs about $247.75 when you apply for your card online. That covers three payments: your initial visit, the state card fee, and one re-evaluation before your first certification runs out.
The visit fees go to Horizon Releaf. The card fee goes to the Florida Department of Health. They are separate payments, made at different times.

Why do I pay for more than one visit a year?
Florida limits how long one certification lasts. Your physician must evaluate you again before issuing a new one: at least once every 30 weeks.
One certification may cover no more than three 70-day supply limits (about 210 days), or six 35-day supply limits for marijuana in a form for smoking. So a patient who stays with the program for a full year usually needs one re-evaluation in the first year, and about two in each year after that.
What does a year of renewals cost after that?
After the first year, most patients pay for about two re-evaluations and one card renewal each year. At regular prices that is also about $247.75 a year when you renew the card online.
Returning-patient discounts lower the visit part. For example, a Veteran pays $85 minus $25 for each re-evaluation.

Which discounts can returning patients get?
Returning patients can take one of four discounts off the visit fee, starting with their second visit. Please bring proof of eligibility.

Does insurance cover a medical marijuana card in Florida?
No. Health insurers and government agencies are not required to reimburse any expenses for the medical use of marijuana, and marijuana is not reimbursable under Florida workers' compensation. This comes from Article X, Section 29 of the Florida Constitution and section 381.986(15)(f) of the Florida Statutes.
Certification visits at Horizon Releaf are self-pay. We do not bill insurance.
Where does the state card fee go, and when do I pay it?
You pay the state card fee to the Florida Department of Health when you apply for your card, after the physician enters you in the Registry. $75, paid to the Florida Department of Health (plus a $2.75 convenience fee when paying online)
After the physician enters you in the Registry, you apply online in the Medical Marijuana Use Registry (or by mail) with a photo, proof of Florida residency, an e-signature, and the $75 fee. Online applications average about 10 business days to approve. Since July 27, 2026, you can only apply or renew if you have a current or scheduled certification with open orders in the Registry. If you do not qualify at your visit, you never pay the state fee.
What is not included in these costs?
These figures cover certification and the state card only. The cost of medical marijuana itself is set by treatment centers, and we do not recommend any product or dispensary.
Caregivers apply for their own caregiver ID card through the Registry, separate from the patient's card.
